Support from The Palette Fund will ensure that HMI's Café (year-round food program that includes nutritious snacks and daily hot dinners served to the youth every weekday at 6:00 PM and lasting for a full hour) is adequately funded to feed all youth who attend for the next three years. These services will help draw youth into HMI’s space where caring staff can build relationships with them to help them overcome the challenges that are keeping them on the streets. The Palette Fund will also enable a case manager and a sustained patient navigation model to provide individualized service plans to link youth to critical services and programs such as mental health counseling, referrals for legal, medical, outreach services and support for homeless/street LGBTQ youth, substance abuse treatment, GED classes and tutoring, job Readiness training and increasing their access to emergency shelter and/ or transitional living programs.
